Requesting Plan Information
Understanding your NDIS plan doesn’t have to feel overwhelming. One small action can make a big difference by knowing exactly what’s in your plan and why it was funded. Most participants only read the plan itself — the goals, the budgets, and the dates. But behind every plan is a record of how the NDIA reached those decisions. These records can help you understand what worked, what didn’t, and how to better prepare in the future.
Requesting Information from NDIS
Did you know that you can request a copy of the following:
A complete breakdown of your NDIS plan.
Meeting notes from your NDIS planning meeting.
Planner justification on decisions relating to your support outcomes.
All documents that have been uploaded to your NDIS portal.
After every plan meeting, the NDIA records internal notes, including:
What you talked about
What evidence you provided
What concerns the planner had
Why specific supports were funded
Why other supports may have been reduced or not included
You’re allowed to request these notes at any time. This is one of the most underused rights participants have and it’s incredibly powerful.
Why does this matter?
Understanding your planning conversation helps you:
1. Prepare stronger evidence
You’ll know exactly what the NDIA questioned, valued, or overlooked. This helps you brief your allied health team properly before your next review.
2. Avoid funding surprises
If your last plan dropped supports unexpectedly, these notes highlight the reasoning behind it.
3. Address misunderstandings early
If something was misunderstood or recorded incorrectly, you can correct it now and not in 12 months.
4. Advocate with confidence
When you know how decisions were made, you can speak up more effectively, whether in a check-in, review, or appeal.
5. Save time at your next meeting
Clear notes help you and your Support Coordinator avoid repeating old information and focus on what actually matters.

Complete an online form through the NDIS website:
